I went to the office of the Professional Regulation Commission or PRC to renew my license identification card and update my marital status as well. As far as I remember, it was my fourth license after passing the nursing board exam in 2001. I waitedIf you'll ask me if there's a difference in renewal process for change of status, I would say yes. Before you go to PRC, make sure you have the needed documents whether you're applying For PRC License rewewal or change if status.
Requirements for PRC license renewal and change of status:
So I went to PRC in Morayta with my usual documents plus Marriage certificate, NSO copy. The first thing you should do is get an application form located near the main entrance. Fill-up the form
1. Application/PRC renewal form.
2. Marriage certificate original from NSO and a photocopy (at least two copies)
3. PRC ID and a photocopy of it.
4. Passport size picture with white background and name tag (2 pcs)
5. Pen and paste
Proceed to the 2nd floor, at the 'Legal' department located on your right. Ask for the form for change of status and fill-up the form.
Go to the Legal department and have your document notarized. I think I paid Php50 for notarization
Proceed to window 16 complete with all your documents, PRC renewal or change of status application form, marriage certificate, PRC ID, passport sized photos and photocopies of the documents.
When finished at Window 16, they will tell you to proceed at records section and then payment. When done, you will have to go back to window 16 for final instruction like when are you going to return for the release of your new license. It took about two months before my license was release. It's okay sinceI'm not yet needing my new license.
